Request for a demonstration installation of CrossWords

If you would like to order a demonstration copy of CrossWords:

1 - Read this form and the CrossWords License Agreement.

2 - Sign and date the form, and fax it to Levit & James, Inc., at (703) 779-0304.

3 - Upon receipt of the signed form, we well send you an e-mail with a demo license
and a link to where you can download the software.

I wish to order a demonstration copy of CrossWords software from Levit & James,
Inc. I certify that I have read the attached CrossWords License Agreement, and will
abide by all terms and conditions therein. In addition, I agree that I can use the
software for only 30 days, starting on the day of receipt, and that I am limited to
converting up to 500 documents with the software. At the end of thirty days, I must
completely remove the software from all computers it has been installed on, or
purchase the software from Levit & James, Inc., or one of its authorized resellers.

CrossWords Customer Information Form

To assist you with the configuration and installation of CrossWords, we request that you schedule
a planning meeting with our technical staff. On your side this should include the appropriate person(s)
on your technical staff as well as your integrator and/or consultant if you are using one for this project.
Before the planning meeting you and/or your consultant or system integrator must:

1. Fill in the information on this form, and fax it to (703) 779-0304;

2. Carefully Read the CrossWords Documentation, and prepare any questions you may have;

3. Call Colin Lowry, our technical support manager, at (703) 771-8185 to schedule the planning meeting.

On the call, we will review how CrossWords works, the details of configuration, and will discuss
the setup, testing, and conversion processes. We will answer any questions you have, about
CrossWords, its features and options, or our installation and support policies.
